Important details for Infant Baptism

We are delighted to welcome your family to our parish and to share in the joy of your child's baptism. A member of the baptism team will contact you shortly and accompany your family throughout the process.

To start the process, please fill in this form and submit it. A copy of your child's birth certificate, one of the parents' baptism certificates and one or both godparents confirmation certificates are required to schedule the baptism. If you are unable to attach them to the form, please send them to: office@st-luke.ca.

N.B. Proof of baptism and confirmation from at least one parent and godparent is required before a date is confirmed.

Once the necessary documents have been received, a member of the team will contact you to book your date of preparation for baptism and the date on which your child will receive the sacrament of baptism.
